Forms requiring parent signatures

REQUIRE STUDENT, PARENT AND WITNESS SIGNATURES

Student and parents of students currently in the process of applying for Off-Campus Study, may download the approriate required Parent Statement/Release Agreement:

• If your son/daugher is studying on a Colby Program (Colby in Dijon, Salamanca, St. Petersburg, or Bigelow Lab OR the Dartmouth Engineering Program)

COLBY PROGRAM Statement & Assumption of Risk form (Original signatures required)

• If your son/daughter is studying on a Non-Colby Program (all other programs)
  

NON-COLBY PROGRAM Statement & Release and Hold Harmless form (Original signatures required)
