Colby Application for Approval

DUE  BY MARCH 1 (IF PETITION) OR MARCH 15
  • This is an online application process to gain Colby approval for your off-campus study plans.
  • Each application is SPECIFIC TO AN OFF-CAMPUS PROGRAM OR UNIVERSITY abroad
  • There are TWO DIFFERENT FORMS: one for Colby-run programs and one for non-Colby programs. 
  • For COLBY PROGRAMS this form serves both as permission to study abroad and application to the program
  • For NON-COLBY PROGRAMS the form serves only as Colby Approval for your off-campus study plans. You must also apply directly to  the program or university abroad itself).
  • You must complete and submit ONE APPLICATION FORM FOR EACH PROGRAM/UNIVERSITY to which you intend to apply.
  • You may submit TWO APPLICATIONS for Approval.
  • The PETITION PROCESS in included within each application form. A MEETING WITH OCS IS REQUIRED.

You must:

  • MEET WITH YOUR ADVISOR(S) AND DEPARTMENTAL OCS LIAISON(S) before submitting
  • MEET WITH OFF-CAMPUS STUDY IF YOU INTEND TO PETITION
  • Request APPROVAL FOR AN OFF-CAMPUS COURSE TO COUNT TOWARDS YOUR MAJOR/MINOR/DISTRIBUTION REQUIREMENT. You should bring or email the professor a course description or syllabus for the course.
  • Gather witness & parent signatures on the PARENT STATEMENT & RELEASE AGREEMENTS (downloadable below and from the application).  ONE FOR EACH PROGRAM.

  • COLBY PROGRAM: Use ONLY for Colby-run OCS programs (i.e., Colby in Dijon, Colby in Salamanca, Colby in St. Petersburg, or Colby at Bigelow Laboratory)
             Application Form   This application serves as your application to the Colby program off-campus.

    Please provide OCS with
  1. A copy of your Dartmouth application (or complete the Non-Colby Application form)
  2. Course approvals form
  3. Colby Assumption of Risk form (original signatures required)


PETITIONS

If a petition is required you must have a MEETING WITH OCS and
submit your application with petition included by March 1.  Review Petition Guidelines.


COURSE APPROVAL FORM

Check each academic department's process for obtaining approval for off-campus study courses to count towards your major(s) or minor(s) or distribution requirements.


REQUIRED COURSE LOAD

Check the required course load for your program/university.

 
RELEASE FORMS to be signed by STUDENT, PARENT AND WITNESS
  1. DOWNLOAD OR EMAIL the appropriate Parent Statement/Release Agreement here and
  2. PRINT IT
  3. SIGN IT
  4. RETURN THE ORIGINAL BY MAIL TO OCS at the address indicated on the form.

FOR COLBY PROGRAM (i.e., Colby in Dijon, Colby in Salamanca, Colby in St. Petersburg, or Colby at Bigelow Laboratory) use COLBY PROGRAM Statement & Assumption of Risk form (Original signatures required)
 
FOR NON-COLBY PROGRAM (all other programs) use NON-COLBY PROGRAM Statement & Release and Hold Harmless form (Original signatures required)
