Student & Parent Statement/Release Agreement

Student and parents of students currently in the process of applying for Off-Campus Study, may download the appropriate required Student & Parent Statement/Release Agreement: 


IF STUDYING ON A COLBY PROGRAM 

If you / your son/daugher is studying on a Colby Program (Colby in Dijon, Salamanca, St. Petersburg, or Bigelow OR the Dartmouth Engineering Dual Degree Program)

Please use COLBY PROGRAM Statement & Assumption of Risk form        

  IF STUDYING ON A NON-COLBY PROGRAM

If you / your son/daughter is studying on a Non-Colby Program (all other Colby-approved programs)

Please use NON-COLBY PROGRAM Statement & Release and Hold Harmless form
  1. Download Form,
  2. Print Form, 
  3. Sign Form (Original signatures required),
  4. Return the ORIGINAL BY MAIL to OCS at the address indicated on the form
(Scanned or faxed copies are not acceptable).   Student and parent signatures may be on different forms.
