1. On your Moodle course site, turn editing on
  2. Click the “Add an activity” drop-menu and choose “Feedback”
  3. On the new Feedback activity page:
    1. Name it something like: “Remote Learning Environment Survey”
    2. Scroll down and open the “Question and submission settings” section, click the drop-menu for “Record user names” and select “User’s name will be logged and shown with answers”
  4. Click the “Save and display” button at the bottom
  5. On the next page, click the “Templates” tab
  6. Click the drop-menu in the “Use a template” section and choose “Remote Learning Environment Survey”
  7. Click the “Save changes” button
  8. All set, you can edit the Feedback activity if you’d like to add additional questions of your own
