Registration with Support Services In order to be considered for accommodations through Colby College, students must first submit documentation of the disability according to College guidelines and meet with Dr. Joseph Atkins, Assistant Dean of Students, to discuss accommodation requests. Students who would like information about the registration process should contact jeatkins@colby.edu. Process for Requesting Accommodations for Support Services: 2. Student completes a voluntary disclosure form. Click on the form associated with your disability to download a copy of the guidelines for Documentation of Disability: Guidelines for Documentation of a Specific Learning DisabilityGuidelines for Documentation of Attention-Deficit/Hyperactivity Disorder (ADHD/ADD) Guidelines for Documentation of Psychological Disorders Documentation of Physical Disability and Chronic Health Issues 4. Student meets with Dean Atkins to determine eligibility under College guidelines and discusses accommodation process. Once eligibility is determined students do not need to meet with Dean Atkins again unless accommodation changes are being requested. 5. If determined eligible, student requests accommodation and fills out and returns the request for accommodations (this is mailed to the student at the start of each semester and must be filled out within the first two weeks of classes each semester). 6. Dean of Students Office prepares letter of accommodation.7. Students pick up letters and hand deliver to faculty. Registering with Support Services Without Requesting Accommodations: 1. Student completes a voluntary disclosure form. 2. Student provides documentation of disability according to College guidelines 3.
