Proton   Pump Inhibitors (PPI)

Medications used to treat   heartburn, ulcers and stomach acid reflux.  These medications are   effective in the treatment of acid-related gastrointestinal disorders,   including gastroesophageal reflux disease (GERD) and peptic ulcer disease   (PUD)

Aciphex, Protonix, Prilosec   40 mg, and Zegerid, Pantropazole will no longer be covered benefits.

Nexium, Prevacid
Omeprazole 

OTC Products:

Prilosec OTC

Non-Sedating Antihistamines (NSA)

Medications used to treat seasonal allergies, hay fever or other   allergies.  Symptoms commonly include nasal congestion, a runny nose,   sneezing, itchy and watery eyes, and skin rashes.

Allegra/Allegra D, Clarinex/Clarinex D, Semprex D, Xyral and Zyrtec/Zyrtec D will no longer be   covered benefits.

Rx Products:

fexofenadine (generic version of   Allegra)

OTC Products:

Claritin OTC,   Alavert, Zytrec OTC

Ophthalmic   Allergy Agents

Medications used to treat eye irritation related to seasonal   allergies, hay fever or other allergies.  Symptoms of allergic   conjunctivitis include itching, redness, tearing and lid swelling.

Alamast, Aloril, Alomide, Elestat,   Emadine, Optivar, and Patanol will no longer be covered benefits.

Rx Products:

cromolyn sodium,  ketotifen   (generic version of Zaditor) (where covered)

OTC Products:

Zaditor, Naphcon A, Opcon A
