Medical records are important documents in the administration of healthcare. If part of this record should change, please let the Health Center staff know so that these changes can be recorded.
Medical records are confidential documents. The staff at the Garrison-Foster Health Center ensure that all patient information is kept private. Medical records will not be released to any third party without the explicit permission of the student
Access to / Release of Medical Records
Students, who wish to access their medical records can do so at the Health Center by appointment, between the hours of 8 am and 4 pm, Monday through Friday.
Students may request a copy of their medical record by signing a written release. This may be delivered in person or by:

All full time students are required to have health insurance coverage while attending Colby. All students are automatically enrolled in the Commercial Travelers Health Insurance and billed the annual premium. A student may waive participation in the Commercial Travelers plan by documenting that he or she has comparable coverage under another insurance policy. Documentation of coverage must be provided annually by fully completing the appropriate online Waiver Form. This form must be submitted by August 1st, prior to the beginning of the academic year. All students are sent an e-mail in the spring with instructions and a link to the on-line form. Parents can access the Health Insurance registration page through the Parent Portal at myColby. No refunds will be granted after September 15th of the academic year.
