Both the HMO and Comp-Care plans offer a prescription drug card program. Available from participating pharmacies - locally and throughout the United States - generic prescriptions cost $10, brand-name (formulary) prescriptions cost $25 and optional brand name (non-formulary) prescriptions cost $40 for a supply of up to 30 days. Certain maintenance-type medications are available in supplies of up to 90 days ($20 generic, $50 brand name (formulary) and $80 optional brand name (non-formulary) when purchased by mail-order. Anthem has a "step-therapy" program for certain prescription drugs. "Step-therapy" means that Anthem will require physicians to prescribe less expensive "first line" medications before moving to a more expensive "second line" medication. If the "first line"medication is not successful, the physican will be able to prescribe a "second line" medication. At present, four prescription drugs are involved - Celebrex, Advair, Vioxx, and Bextra - but additional medications may be added in the future. The Drug Formulary Quick Reference Guide consists of 80% of the most often prescribed drugs. This guide will help you in determining the co-pay of most prescriptions, if you do not find your prescription in the Quick Reference Guide you may check the Complete Anthem Prescription Formulary, or call Mary Nelson, Benefits Specialists, x5503, for assistance.
