Change in Personal Information Form: Use this form to report address, marital status, spouse/partner, dependant, or other status changes.
Application for Dependent Tuition Subsidy: Eligible staff may print this form and submit to the Office of Personnel Services along with a copy of the tuition bill from the institution the dependent will be attending.
Medical Reimbursement Claim Form: Employees who have flexible spending accounts may print and use this form to submit a claim for reimbursement to Group Dynamic Inc..
Application for Employee Tuition Reimbursement: Eligible staff may print this form and submit to the Office of Personnel Services upon the successful completetion of each course. Form must be accompanied by payment receipt and grade report.
Campus Directory Form: Use this form to update, change, or verify information that you want included in the official Colby Directory.
Label Request Form: Use this form to request mailing labels for Administrative and Support Staff.
Attendance Form: Use this form to record vacation, sick days, holidays, and other attendance information for Administration and Support staff.
Employee Leave Cards: Use these cards to request and report days taken for vacation, sick leave, family illness, floating holidays, bereavement, and comp time for Administration and Support staff.
Performance Appraisal Form A: An outline of the six general areas; a review of job descriptions and performance standards; a review of last year's goals and accomplishments; a review of personal, professional, and skill development activities; strengths and areas to improve; an overall assessment of performance; and, goals for the coming year.
Performance Appraisal Form B: Focuses on specific job responsibilities (to be filled in for each employee).
Performance Appraisal Form C: A simplified form allowing the supervisor to assess 10 performance factors in three major categories.
Appraisal Guidelines for Supervisors : The March 1, 2006 memo from Douglas Terp and Richard Nale details important information about key dates and the appraisal process itself to assist Supervisors in completion of the appraisal forms.
