Assistance and Service Animals
At Colby we recognize the need for assistance and service animals as an accommodation to ensure access to the full Colby experience. In tandewm with Campus Life, the Office of Student Access and Disability Servives works to support a healthy, safe and successful residential experience for students and their assistance or service animals.
Please read ourService/Assistance Animal Policy to better understand your responsibilities with regards to our policy.
To request an Assistance or Service Animal as an accommodation, submit the following documents to accommodations@colby.edu:
- A Housing/Dining Accommodation Request with Assistance/Service Animal checked.
- Disability Specific Documentation. Our Colby Documentation Guidelines will give you information on what we will need from your providers.
- If you would like to have your documentation sent directly to the office, please complete and submit an Authorization to Release & Exchange Information Form along with your documentation.
- A signed Assistance/Service Animal Contract indicating with your initials that you understand the full policy.
Once your documents have been submitted, our office will reach out to discuss what you can expect before your request is submitted to our committee for review. While requests will be reviewed on a rolling basis, because you are not permitted to have your Assistance/Service Animals on campus with permission, we suggest you start the process as early as possible.
