We’ve simplified the performance review process by consolidating all previous templates into a single, comprehensive Staff Performance Review form. This new, multipurpose form replaces the former separate documents used for hourly staff, administrative staff, probationary reviews, self-evaluations, and the previously used narrative form.
The updated form captures all the essential elements from the previous formats and is designed to improve efficiency and consistency across the review process. It allows both reviewers and employees to navigate the process more easily while maintaining the depth and structure needed for a meaningful review.

Employees choosing to leave Colby College need to submit a written letter of resignation, either in the form of a letter or by completing this form. A copy of the document should be given to the supervisor and the original sent to the Office of Human Resources.
Upon receipt of a resignation document, the Office of Human Resources will send the employee an exit survey, and once completed, contact the employee to schedule a follow up exit interview.

Colby supports employees in a variety of ways, such as the staff gathering, the holiday dinner dance, lobster bakes, ice cream socials, cooking classes, departmental functions, events committee outings, and through many other activities and services.
The College can also recognize and reward employees beyond normal salary and benefit programs for outstanding performance. These awards are only for extraordinary achievements that far exceed assigned responsibilities.
In light of updated IRS guidance, Colby must report the value of any gifts an employee receives above $25 on an employee’s W-2 form. An Employee Gift Form must be completed and approved by a senior staff member before the gift or gift card can be issued. The form must accompany any request for reimbursement, and a copy must be submitted to the Payroll Office for any gift over $25.
