Print (where applicable) and fill out the form, obtain signatures as required, and return it to the Registrar’s Office (Eustis 201), or mail it (4620 Mayflower Hill, Waterville, ME 04901), or fax it (207-859-4623). A completed form that has been scanned may also be sent as an attachment to an email to [email protected]
